Process of Admission...
In following the guidelines set by the State of Alabama for school children, a child enrolling in the CHUM Preschool program must be two, three- or four-years old by September 1 of the year of his/her enrollment.
All Forms required for enrollment should be completed and returned to the Preschool Office prior to the first day of attendance. These forms include:
1. Registration Form( download pdf)
2. General Information Form
3. Emergency names/numbers
4. Transportation Plan
5. Medical Release Form
6. Blanket Field Trip Form*
7. Blue Immunization Record**
*The Blanket Field Trip Form for the 2 and 3-year-old Preschool children will allow them to go for walks to the empty lot next door to pick up fall leaves, acorns, etc. for art collages.
**THE BLUE IMMUNIZATION RECORD IS REQUIRED TO BE ON FILE AND MUST BE UPDATED WHEN NECESSARY. Children's files are kept confidential and are kept in storage for a period of two years after the child leaves our program.
